CHECKLIST OF REQUIRED
INFORMATION
BRING THE
FOLLOWING WITH YOU: |
- COPY OF YOUR HIGH SCHOOL
DIPLOMA OR GED.

- COPY OF YOUR
VALID DRIVER'S LICENSE.

- BE PREPARED TO FILL OUT AN
APPLICATION - HAVE YOUR EMPLOYMENT INFO WITH YOU!

- BE PREPARED TO PAY FOR AND
COMPLETE THE WRITTEN JOB SKILLS TEST. $15.00 FEE REQUIRED.

- IF YOU ARE A GRADUATE FROM A
COMMUNITY BASED ORGANIZATION (TrANS, ETC.) - PROVIDE A COPY OF
YOUR COMPLETION CERTIFICATE.

WHAT HAPPENS AFTER THE INFORMATION
SESSION?
We will verify the
information you provide at the information session. After we verify your
information:
-
A "Letter of Eligibility"
will be mailed to you. You should leave a copy of the "Letter of
Eligibility" when you apply with
contractors. If a contractor would like to hire you, they should fill
out the back of the
"Letter of Eligibility" and fax it to the training site. We
will then contact you to schedule you to come to the training site for
an Orientation to sign your contract with the State of Wisconsin
Bureau of Apprenticeship. You CANNOT
be considered an apprentice until after you sign a contract with the
State of Wisconsin Bureau of Apprenticeship.

will accompany the
"Letter of Eligibility."

You will be placed on
the list to be called for jobs as they become available.
The
"Letter of Eligibility" is good for ONE year.

RENEWAL INFORMATION
EFFECTIVE
IMMEDIATELY current
"Letter of Eligibility" holders WILL NOT BE ADMITTED AS AN
OPERATING ENGINEER APPRENTICE until the Job Skills test has been
completed. You may reapply so that you have a current test on file or
request a renewal letter in writing and take the test when you are hired
by a contractor.
